4. Patient Portal Use
- The patient portal is provided for your convenience in managing appointments, reviewing health information, and communicating securely with our practice.
- You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account.
- You must notify us immediately at info@havennexuspsychiatry.com if you suspect unauthorized access to your portal account.
- The portal is not intended for use in medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
5. Protected Health Information (PHI)
- Any protected health information (PHI) transmitted, stored, or accessed through the Services is handled in accordance with our HIPAA Notice of Privacy Practices and applicable federal and state law.
- You should not share your login credentials with any third party, as doing so may result in unauthorized disclosure of your own PHI.

Do not use unsecured channels (such as ordinary email or text, outside of our designated secure messaging systems) to transmit sensitive medical information.
